To apply for Disability allowance (counselling), you will need to be assessed by a doctor who will complete a disability certificate for counselling.
I am a Work & Income NZ approved counselor and can complete this part of the form. Contact Flowers on 027 490 6511.
If you decide to go with another practitioner ask if they are willing to be funded by the disability allowance.
The DA will cover your counselling costs to a maximum of $80.00 a week for adults.
You can call 0800 559 009 to request forms to be mailed out to you or pick up from your nearest centre.
When your form is completed by yourself, a doctor, and your selected counselor it can be submitted to Work & Income NZ.
